As of August 21, 2026, C.O.R.E. Physical Therapy will no longer operate under its current business structure or from our current office suite. While our current practice structure is dissolving, our commitment to providing exceptional, personalized care remains exactly the same.
The good news is that all of us will continue caring for patients in new locations.

Are my records still available?
Yes. Your medical records will remain secure and accessible. If you need copies of your records or have questions regarding record requests, please contact the therapist you will be seeing moving forward using the contact information provided above.
Can I continue seeing the same physical therapist?
Absolutely! You can continue your care with the physical therapist you have always seen.
Will my insurance benefits change?
No. We have always been out-of-network providers, and that will remain the same. Please note that, beginning August 1, 2026, we will no longer be enrolled with Medicare. If you have questions about coverage, reimbursement, or payment options, please contact your therapist directly.
